What we offer

We provide quality care that is holistic and effective. Service users and carers are involved in its planning and implementation as much as possible.

We provide care aimed at early recovery/discharge. It is based on the multidisciplinary team approach and reflects the range of service-users’ and carers identified needs.

Services include:

  • Assessment and diagnosis
  • An appropriate care plan and treatment
  • A physical health check on admission
  • Support to access advocacy services
  • We also offer support for physical and social needs
  • Occupational therapy
  • Psychological therapy

When an inpatient is ready for discharge this will often be supported by the home treatment team for a period providing additional support whilst working alongside the care coordinator.

How you can access our service

GPs, A&E doctors, police, liaising team, self-referral, care coordinators and voluntary sector carers can refer to the service.
