Barnet, Enfield and Haringey Mental Health Trust is delighted to announce that The Care Homes Assessment Team (CHAT) has been shortlisted for two categories Primary Care Innovation of the Year and Community or Primary Care Service Redesign - London and the South at this year’s HSJ Awards, recognising their outstanding contribution to healthcare.

The judging panel, made up of a diverse range of highly influential and respected figures within the healthcare community, have shortlisted CHAT despite the tough competition from hundreds of excellent applicants. CHAT has been selected based on their ambition, visionary spirit and the demonstrable positive impact that they have had on patient and staff experiences within the health sector. 

CHAT is an integrated physical and mental health service supporting the complex needs of residents in Care Homes across Enfield and Haringey. The service has delivered improved rates of end of life experiences in preferred place and reduction of avoidable admissions to secondary care leading to improvements in quality of life and end of life experiences for this population.
