Enfield Memory Service

Enfield Memory Service, on The Chase Farm hospital site provides specialist assessment to people over 65 years and over, who have concerns about their memory getting worse. We provide specialist assessment, diagnosis and treatment, advice and support to both the patient and their family/carer, advice and support to access local and national support services, and Liaison and feedback to your GP.

The Team

Enfield Memory Service consist of; Team Leader, Charge Nurse, Independent Nurse Prescriber, Occupational Therapist, Associate Health Worker, Consultant Psychiatrists, Team Secretaries, Receptionist, Admin Manager and Music Therapist.

What Can I expect When I am here?

Once a referral is received we will offer you an assessment. This takes place at the Memory Service and lasts for up to 1.30hrs. We ask that you attend the appointment with a relative, or friend who knows you well.

We will also refer you for a head scan, either an MRI or CT scan at Chase Farm Hospital.

At the end of your assessment, we will feed back to you. If required we will feed back to you. If required we will offer you another appointment

When feeding back we will discuss a treatment and support plan with you and your relative/friend

We will write to your GP following your assessment

 

Referral Process:

Referral are received from GP's via the Enfield Assessment Service. We will need the results of blood tests, arranged by your GP. This is to help identify any physical cause to your memory problem.
